Who can take part?
Active Together Calderdale and Huddersfield will soon be open for patients over 18 years old who are receiving curative treatment at Calderdale and Huddersfield NHS Trust and have been diagnosed with cancer.
Referrals onto the service can only be made by hospital teams. We cannot accept self-referrals at this time. If you would like to start referring patients to Active Together Calderdale and Huddersfield, please contact us on ActiveTogether@cht.nhs.uk.
