Steps to take part

1. Register your interest

Complete the short form and our team will contact you to explain what the Active Together service offers, the type of support we provide and how referrals work.

2. Speak to your nurse or doctor

Use the information provided to discuss Active Together with your healthcare professional, who can advise whether a referral is appropriate for you.

3. Join your first session in person

Once your referral is received, we’ll contact you to book your first appointment in Harrogate. Our team will welcome you and guide you through what to expect.
