Offering free personalised fitness, nutrition and wellbeing support, delivered by a team of qualified professionals, Active Together in Harrogate helps people to prepare for and recover from their cancer treatment.
Referrals onto the service can only be made by healthcare professionals. We cannot accept self-referrals at this time.
If you would like to refer a patient, please ensure they meet the referral criteria below and then complete the referral form.

Referral criteria
To be eligible for Active Together, patients must meet the following criteria:
- Aged 18 or over
- Resident of Yorkshire
- Able to travel to Yorkshire Cancer Research, Hornbeam Square West, Harrogate HG2 8PA for initial and final appointments
- Have consented to the referral
- Able to follow basic instructions
- Have received a cancer diagnosis or completed active treatment within the past 12 months
- Be on a curative pathway.
What to expect
The first appointment
Following referral, we will contact the patient within 5 working days to arrange an appointment at a time that suits them.
To help the Active Together team understand how best to support them, the team will take each patient through some simple assessments and together they will design a personalised plan to support them through treatment and recovery.
Staying in touch
Throughout the patient's time with Active Together, we will update you by email on their progress and may also be in touch with questions about their care.
