Active Together Mid Yorkshire will soon be open for patients being treated at Mid Yorkshire NHS Trust, who have been diagnosed with the following cancers:

• Lung cancer
• Colorectal cancer 
• Head and neck cancers

Referrals onto the service can only be made by healthcare professionals. We cannot accept self-referrals at this time. If you would like to start referring patients to Active Together Mid Yorkshire, please contact us on activetogether@ycr.org.uk.
