Active Together Sheffield is currently open for patients over 18 years old being treated at Sheffield Teaching Hospitals NHS Trust, who have been diagnosed with the following cancers:

• Upper gastrointestinal cancer
• Lung cancer 
• Colorectal cancer 
• Urological cancer
• Gynaecological cancer
• Head and neck cancer

Referrals onto the service can only be made by healthcare professionals. We cannot accept self-referrals at this time. If you would like to start referring patients to Active Together Sheffield, please contact the service delivery team by emailing sth.activetogetherservice@nhs.net.
