Referral instructions

1. If you would like to take part in Active Together in Harrogate, you will need a referral from anyone involved in your care at your GP practice or cancer treatment team, which may include your Cancer Nurse Specialist (CNS), oncologist, doctor, physiotherapist, radiotherapist, surgeon, or occupational therapist.

   Speak to them at your next appointment and let them know you are interested in being referred to Active Together in Harrogate.

   You can also share the letter and postcard to give them more information.

2. If you book appointments online:

   You can upload the letter or postcard when you send a message to your GP or request an appointment.

    

3. Once your referral is sent:

   The Active Together team will contact you to arrange your first appointment.
