Who can take part?
Active Together Calderdale and Huddersfield will soon be open for patients being treated at Calderdale and Huddersfield NHS Trust, who have been diagnosed with cancer.
Referrals onto the service can only be made by healthcare professionals. We cannot accept self-referrals at this time. Healthcare professionals can find out how to make a referral here. If you're not eligible to take part, you can still explore our resources section.
