Who can take part?

Active Together is currently open for patients being treated at Mid Yorkshire Teaching NHS Trust, who have been diagnosed with the following:

Lung cancer
Colorectal cancer
Head & neck cancers
Upper gastrointestinal cancers
Haemotological cancers
Urological cancers

Referrals onto the service can only be made by healthcare professionals. We cannot accept self-referrals at this time. Healthcare professionals can find out how to make a referral here. If you're not currently eligible to take part, you can still explore our resources section.
